If you need to reschedule please respond with a few preferred days/times. 

When you're scheduled, you will be sent a second email confirmation from our HIPAA compliant scheduling software Acuity with instructions for providing new or updated medical history (please fill out the survey form), and you can upload your records or ID here too if needed.  

There are legal updates and reminders sent at this time, so please notify us if you do not receive this confirmation email. 

Please remember that the ID card expiration date is separate from your medication renewal appointments.  ID card renewals must be done with the Department of Health, and instructions can be found at http://knowthefactsmmj.com/
